What should I bring with me to my first visit?

On the first day, please arrive at least 15 minutes prior to your appointment to fill out the appropriate paperwork. You will need:

    • Insurance card
    • Driver’s license
    • Co-pay (if applicable)
    • Prescription (if applicable)

For your convenience, we accept cash, checks, and credit cards. Please make checks payable to: Full Range Physical Therapy, LLC.

What To Wear?

Comfortable clothes and sneakers are best for the initial evaluation and treatment sessions. Expect about 60 minutes for each session.
